You are on a pretty average dose of Topamax and yes, the side effects are usualy more obvious as you increase mg. If Topamax is working to control your migraine, but you just can't function with the side effects then maybe look at those other anti-seizure meds. I am just a medical student who has migraines, not a Dr yet, but I went through this very same thing. There is Neurontin, Zonegran, and Keppra that you can ask about. I am currently on Keppra, but a lot of docs don't have experience dispensing these meds for migraine. I had to go to the Diamond Headache Clinic to find relief. Once the doc found out I had success with Topamax she stayed in that drug family to help me find relief without the side effects. Depakote made me fat and Topamax made me stupid. Do some of your own research and bring questions to the neurologist about these specific drugs. If you feel like they don't have the experience or don't listen to you, maybe you do need to go to a headache center. I am not sure where you are located but sometimes its hard to find a "headache specialist" who stays up to date on treatments. Good luck to you.
