I have had a chronic headache (24/7) for 2 yrs. It is on the right side on my head, mostly on top. My doctor prescribed flugin hydrocholide (spelling) that helped for a few months, but it came back. I had a MIR done and it showed nothing. Noise and smells intensify the pain. The neurologist says it acts like a migraine but is not a migraine. I went on vacation in May of this year and the headache went away until the end of August. The pain now is a sharp pain. Before May , it would change everyday from a pressure to ice pick to sharp pain. Any thoughts?

If you haven't had a chance to peek at the start folder, I encourage you to take a few minutes and do that. There are all kinds of tips and pointers for making your forum experience better there.

As for the head pain you're experiencing, a lot of it does sound like things we associate with migraine, and most of us can relate to noise and light sensitivities, one-sidedness, and clean MRI scans. It sounds like the doctor you saw is convinced that it's not really migraine, but that doesn't help guide your treatment if he won't commit to an alternate, firm diagnosis. That's what you need here. We're not qualified to diagnose here, so if you're not happy with the results from this doctor, the logical next step would be to find a new and different doctor, preferably a migraine specialist, and get a second opinion and a firm diagnosis. We can't begin to guess, unfortunately.
